Community Aged Care Packages (CACP)
CACPs program is a flexible service funded by the Department of Health and Ageing and allocated to Adventist Aged Care to assist older people with low care needs to remain in their own homes for as long as possible.
Eligibility is determined by the Aged Care
Assessment Team. A copy of the Aged Care Assessment Record (ACAR) must be provided with a copy of a completed application form avail-able from the administration office.
A flexible range of services can be offered to meet your needs. They include:

Personal hygiene,

Medication management,

Light housework,
-
Transport to any nearby medical
appointment or shopping,

Meal preparation,
-
Emotional support,
-
Installation of emergency alarm monitoring systems.
An individualized care plan is developed by our team in consultation with you and your family to ensure it meets your needs.

The service is provided Monday to Friday with negotiated short visits on weekends for some services.
A weekly fee for services is payable depending on your level of income as prescribed by the Department of Health and Ageing. This will be discussed with you at an initial no obligation meeting.

If you are not eligible for CACPs service or do not need regular services, we can still assist you with the same flexible range of services as for CACPs.
A fee for service will be determined in
consultation with you prior to the providing
assistance.
